Hello everyone I am a male and currently 20 years old. I have been experiencing pssd for about 5 months now when I cold turkey stopped taking my SSRI's after taking them on and off for about a year and a half. I have taken Zoloft and Prozac(Fluoxetine) during this time period. After stopping, some of my symptoms included genital anesthesia, very mild ed(can keep an erection but only with constant stimulation), loss of libido, decreased morning erections, and general decrease in boners throughout day to day. So far I have experimented with Wellbutrin, Trazodone, Ketamine, SJW, Zinc, Horny Goat Weed, Maca, Yohimbine, Vitamin D, GNC ArginMax. Ketamine gave me some improvement as did trazodone but I recently tried DIM and I'll explain why in a second. After trying so many things with little to minimal improvment, specifically targeting 5h1ta autoreceptor and my testosterone levels, I decided to go to my doctor and get my free and total testosterone levels checked to make sure there were no issues in that aspect. My total testosterone came out to 902 ng/dl (ref range 280-950 ng/dl) and my free at 33.4 ng/dl(ref range 5.25-20.7). Both of these values are very high. Note that at the time I was also taking sexual supplements such as horny goat weed, maca, yohimbine, arginmax etc which bloated my test result. I didn't realize this until later on and beleieved that testosterone was not the problem. I still had no improvement at about 3 months so I decided to go into my doctor and get a full hormone panel done. I tested TSH, E1(Estrone), E2(Estradiol), FSH, Luteinzing Hormone, Prolactin,Progesterone, Free and Total Testosterone, SHBG(Sex Hormone Binding Globulin), and DHEA. Everything came out completely normal except for the main sexual hormones(some of which were slightly elevated) E1, E2, and free and total testoterone. My E1(Estrone) was 47 pg/ml(Ref range 10-60 pg/ml) and E2(Estradiol) was 57 pg/ml(ref range 10-40 pg/ml). That right away told me that I have high estrogen levels due to my E2 levels being elevated which can cause ed, loss of libido and decreased sensation. To add to this, my total testosterone was strikingly lower than last time coming in at 408 ng/dl (ref range 280-950 ng/dl). My free testerone came out fine but still much lower than my first test at 15.5 ng/dl(ref range 5.25-20.7). As a healthy male 20 year old this did not seem normal at all. No one in my family has ever had low testosterone issues especially at my age. Note that during this full hormone panel test I was not on any testosterone supplements and the only medication I was taking was wellbutrin for my depression and possible solution to pssd. My theory is that the SSRI's caused my estradiol levels to increase which in turn caused my testosterone to decrease as explained in this article https://www.ncbi.nlm.nih.gov/pubmed/28179152. "Furthermore, all SSRIs relatively increased the estrogen/androgen ratio, indicating stimulating effects on the aromatase. Our study demonstrates the potential of SSRIs to interfere with steroid production in the H295R cells around Cmax levels and indicates that these drugs should be investigated further to determine any hazards for the users.". Also after reading this second article https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4854098/ I gained a lot more knowledge on why my hormone levels(specifically testosterone and estrogen) could be causing my ed. At one point in the article it says that males with high estrogen and low testosterone are at the highest risk for ed than any other combination. That being said I am taking DIM which is supposed to act as an aromatase inhibitor to decrease the conversion of testosterone to estradiol. I have been taking this for about a month now and at the 2 week mark I decided to go get my estradiol(E2) levels checked and they were 48 pg/ml 9 lower than my previous estradiol(E2) result. So obviously something is working. In terms of improvement, I've had slightly increased sensation and libido although it's nowhere near perfect. I will continue to take DIM daily and eventually supplement testosterone to get the levels up to something more reasonable(at least 600). Will keep you updated on my progress. Nonetheless, what do you guys think about my theory as to what might be going on and how ssris may have caused this increase in estradiol levels? Thanks for reading!
